I think it is about a man who wasn't ready to be a father and parted ways with his pregnant girlfriend because of it. Now he is ready to be a father and he wants to go back to her and meet his daughter but he can't because she has found a father for Sierra. the only reason I don't think he left his family after his daughter was born is he admits to not knowing her her name or perhaps if she has a nickname when he admits "and I wonder what the call her." He also confesses a lot of guilt in not knowing her when he says "I'll never know, know who you are, and I don't deserve to" which furthers the idea that he has never even met his daughter(that is my favorite line in this song).
