Some people are saying that its about a guy prostituting his girlfriend to get cash for heroin, but I think it's about him prostituting himself. The lines like "He wants nothing less But to wear a little dress" refer to his transexuality and the final line "I need someone to save my ass" refers to his need for someone to save him from prostitution. I think he is saying "She" because he thinks of himself as a "she" and the 'like heroin' is how he feels when he is her. This is just my interpretation.
