its a story of a drug induced psychosis. Anyone who has or does take drugs will understand. Shaking could be an OD or just the jitters. Feathered left arm is him refering to cutting, using the word feathered in a decorative way "he feathered his own nest". In essence the song is about the sober him trying to come to terms with his intoxicated counter self

I really love this song. It's so dark, so deep, and just pure poetry. I also feel like I can connect to it on some level.
As for the meaning, I really think it's about two people (and there's nothing physical/sexual about it, though it sort of seems like there would be). One of them has something like schizophrenia or bipolar disorder/manic depression or multiple personality disorder. The other one has their own problems (sounds like epilepsy from the first few lines). As the singer beings to care and learn more about the other person, he realizes more about himself--and "himself." How his destructive and caring sides are tightly intertwined; he connects them with this "wire through [his] cheek" and tries to reconcile them into his one person. It also seems as the more he learns about the other person (the epileptic one), the more he attaches his sanity to them and what they share (though the wire down his sleeve).
I may be over-analyzing it, but there seems to be a lot woven into this song. It all connects into one beautiful piece.

The beginning could be about an epileptic seizure, however my best friend is very epileptic and this song is to dark to describe his epilepsy. If you have ever seen "A Beautiful Mind" then you should be able to recall the seen in which John Nash is buckled to the bed during his procedure. So, I think this song is about being a schizophrenic. Here is some of my evidence:

Also I think the wire symbolizes how over time as his schizophrenia grows, his acceptance of his other "self" legitimates, and an inseparable bond is formed between himself and "himself".

This song is NOT about rape and more than likely doesn't even have anything to do with a physical relationship. I believe the part that sounds physical at the beginning is about someone who is epileptic.

This song is incredible. It's about trying to help someone when you're messed up and can't even help yourself. It both leads into and stems from many other songs on this album.

The lines "And where crying isn’t secret, It’s the art of how we grieve" and the idea of crying being an art, definitely fits the whole sadist theory. It's interesting how protective, yet destructive he is within this song.

The lines following the first chorus are really interesting. Like he's two different people--the protector and the destroyer.

Makes me think of this whole innocent inquiry of "Did I do that?" I love the description, the way he seems to have realized that he's destroyed something angelic and beautiful.

The song is about sado-masichism. Yes, it also attacks society a bit, but it mostly the singer trying to detirmine why he has these impulses to hurt and be hurt. An amazing songwriter, love this entire band.
