My interpretation, is that this is an alienated comment on the selfish, materialistic world, which refuses to deal with its problems and shoves them on to others over the "decades". Just as with the song 'Isolation' in the lines "Surrendered to self preservation, from others who care for themselves".
Now here are the people (young men) who do care to take on the burden, but; "Where have they been?"
Yeah, psychedelics can do that to certain people... And "thank God" for those people..
And it is up to everyone who does see and care to not let others get away with their apathy, and confront them by asking; "Where have you been!?!"

Think about what difficulties JD/Ian was experiencing at the time this song was written...
.trying to break onto the established music scene but were incredibly anti-establishment.
.trying to say/do something astoundingly relevant but were misunderstood, under-represented and underpaid.
.on the flip-side, as emerging icons of the music underworld, feeling like they had to 'represent' or present society's shortcomings, not a popular thing with the powers that be.
...'drag themselves in' 'portrayal of the trauma' ...onto the live stage, recording studio, television and bare their souls time after time, with no guarantee that anyone was really 'getting it'.
