This song truly portrays a beautiful sentiment.

I love the idea of the thought "everybody knows that you'd break your neck to keep your chin up"

To me, this seems like he's telling her how much he appreciates her. He sees how hard she works ("break your neck" is an expression meaning that you're working hard.) to remain positive ("keep your chin up" means to stay positive.)

This is an incredibly pretty song, and I'd reccomend it to anyone.
