My interpretation of this song is it's about someone who's went through a horrible event, maybe losing all of their family or something, that might've changed their perception of things. This is mostly due to the word "lobotomy." Literally, a lobotomy is a surgical procedure that involves cutting some nervous tissue and things from the brain, but maybe in a figurative way, it could mean something that changes someone's way of thinking or something.
