It fits the description completely. Its about a Japanese girl named Sayaka who goes to study at a music school in London to study music due to her talent in piano. There she meets a british guy called Adam. They fall in love but due to an unfortunately sickness, Sayaka dies during the last quarter of the moon. Before she dies she tells Adam to find her again in the next life and they would fall in luv again. Adam soon after commits suicide to be with her in the next life. 19 years later, when the moon is a last quarter again, a girl named Mizuki is the reincarnation of Sayaka, and the same age when she died. However, even though Sayaka was reincarnated, Adam never was because he commited suicide and thus he interacts with Mizuki through his ghosts and memories. She falls in luv with him over again, but slowly starts to realize that she can never be with him, because the part of her that’s Mizuki needs to have a chance to live her own life instead of going with Adam. The song matches this completely.
