He's being a bit ironic, and myabe he's singing from the perspective of the other person - that would be very his style. Milk It is similar to Scentless Apprentice, it has a specific drums. "Look on the bright side, suicide; lost eyesight, I'm on your side!" It's a beautiful song. Nirvana comes off as too rough and hard for most people. Songs like "Milk It" and "Scentless Apprentice" employ grotesque lyrics - "I am my own parasite; I don't need a host to live; we feed off of each other; we can share our endorphins." It is the reflection of the ennui and anger of a generation; anger specifically at the fact that one is born in the first place, and that one has to be alive to endure pain. "Milk It" paints the mind as the one savior in a seemingly desolate human world with an even more dismal outlook... in the most positive way possible.
