I however have grown up with Metallica. My Uncle Dan used to play/sing it for me as a lullaby. It soon became my security blanket and my parents had to play it on cassette for me to fall asleep. Anyone who has ever had to battle with themselves or a mental illness can understand it. I can't speak for the writers intentions on it. It does simply describe the state of how you feel in an asylum. And as for the moon is always full line. I believe that refers to the systematically numbing routine every night. I know in my experience i never slept because it was always like a full moon night. I was always staring at the sky through the caged windows and the light from the constant suicide checks simply put represents that full moon that lights the sky and can obscure the stars around it. Very well written song. Very thought out.
